Two things you need to do:

Run 'xev' in a terminal and see if it detects a 4th button click.  If it
does, X is receiving the mouse clicks.

Under GNOME (KDE probably has something similar) there's a "shortcuts"
option in the config app.  This has a "capture" facility so you can choose
'ctrl W' and "capture" a 4th button click to make the association.
