I could be wrong, but I think KMail is downloading messages directly
from the remote machine, using either POP or IMAP. It may then itself
write it to the spool.

If you don't have shell access to the remote machine, or it doesn't have
SA or procmail installed, KMail is the first chance you get to touch the
mail.
