That is one of the things I look at when evaluating a company's internal 
culture - do they have a "blame somebody" rather than a "let's fix it" 
culture for handling problems and errors.

By your standards (and mine) many *companies* are unable to be trusted 
to do things right, and/or to do the right thing.
